About the team 
The Ambulatory Services team delivers a broad range of outpatient and support services that contribute to seamless, patient-centred care across the health service. The team encompasses PM@H, Day Therapy, Specialist Clinics, Admissions, Registration and Referrals, the Parkville Precinct Familial Cancer Centre, Clinical Photography, Dental Oncology, Medicare Benefits Schedule (MBS) billing, and Patient Navigators. Working collaboratively across these services, the team supports patients throughout their healthcare journey by facilitating access to care, coordinating appointments and referrals, and providing specialised clinical and administrative support to enhance the patient experience.

•    A strong background in acute care nursing
•    Clinical skills, including:
     o    Comprehensive patient assessment
     o    Management of the deteriorating patient
     o    CVAD (Central Venous Access Device) care
     o    Cannulation

Remuneration
Remuneration as per Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024-2028. RN GRADE2, Year 1 (YP2): $84,188.00 (full-time equivalent - to be prorated for Part Time) - RN GRADE2, Year 1 (YP9): $107,510.00 (full-time equivalent - to be prorated for Part Time) dependent on candidate experience. 1. Management of patient care continuum.

  • Responsible for systematically assessing the health care needs of individuals and for the formulation of a nursing plan of care, its implementation, evaluation of care and analysing variances/responses to care delivery throughout each care episode. 
  • Assesses patients in a systematic way, identifies the patient's health problems and validates nursing plans of care by involving the patient and/or family in the plan of care. Develops appropriate teaching plans for the patient/family.
  • Integrates information from the nursing assessment and from other health care team members.
  • Revise the nursing plan of care appropriately to reflect current or changing status of the patient and evaluates as appropriate. 
  • Initiates referrals and utilises available resources within and outside the Institute for the purpose of comprehensive care to patients and families.
  • Adheres to the principles of defensible documentation and ensures that nursing documentation is complete, up to date, signed and dated. 
  • Throughout the patient's hospitalisation promotes continuity of care, incorporates discharge planning in the nursing plan of care and records a discharge summary in the patient's nursing history at the time of discharge. 
  • Assists in maintaining a safe environment for patients, staff and visitors.
  • Acts as a patient/carer advocate in relation to the patient/carer rights to all aspects of care, knowledge and privacy. 
  • Management of primary patients including patient education and discharge planning 
  • Patient records show documented assessment and plan of care and continuous evaluation of responses to treatment. 
  • Patients (where able) can articulate discharge plans and self- care priorities.

 

2. Clinical Practice, education and research. 

  • Contributes to creating a working environment conducive to learning and professional development. 
  • Responsible and accountable for maintaining own competencies. 
  • Participates in the development of formal and informal educational programs to assist nursing practice. 
  • Acts as a preceptor for new Registered Nurses and for students from affiliating colleges of nursing. 
  • Systematically evaluates own practice and identifies pertinent area of clinical practice, management and education for improvement.
