YOUR ROLE IN THE FUTURE
Registered Nurse Clinical Specialist – PRIORITY CONNECT 2 Trial
Fixed Term, Part-Time | 32 Hours per Fortnight

 

Peter Mac is seeking an experienced and committed Clinical Nurse Specialist to support the implementation of the PRIORITY CONNECT 2 (PC 2) trial within our virtual, multimodal (pre) habilitation hub. This role is central to delivering high quality, evidence based care to patients and their families/carers within a multidisciplinary environment.

 

As a Clinical Nurse Specialist, you will provide expert clinical oversight, act as a positive role model, support less experienced nurses, and contribute to ongoing professional development across the team. Your expertise in perioperative care for patients with colorectal cancer will be instrumental to the success of this important research initiative.

•    Reports to: Research Manager, Division of Surgery
•    Focus: Perioperative care of patients with colorectal cancer within the PRIORITY CONNECT 2 trial

 

About the Role
In this position, you will:
•    Lead the nursing implementation of the PC 2 trial protocol
•    Deliver patient centred, evidence based clinical care
•    Provide expert guidance to nursing colleagues and act as a preceptor
•    Uphold best practices and contribute to a culture of clinical excellence

 

Remuneration as per Nurses and Midwives (Victorian Public Sector) Single Interest Employer Agreement 2024-2028. Clinical Nurse Specialist (YS9) $104,213.20 per annum (full-time equivalent rate - $43,879.24 per annum for part-time hours). To be discussed with Hiring Manager.

Position Summary

The Clinical Nurse Specialist is responsible for implementation of the protocol of the PRIORITY CONNECT 2 (PC-2) trial within the virtual, multimodal (pre)habilitation hub at Peter Mac, providing expert clinical care for patients and families/carers within a multidisciplinary environment. The Clinical Nurse Specialist ensures care provided is evidence based, patient centred and within their scope of practice. As a positive role model for professional practice, the Clinical Nurse Specialist acts as a preceptor to less experienced nurses, is a practice resource for colleagues and demonstrates a commitment to the continuing professional development of self and others.

1. Management of patient care continuum.
•Responsible for systematically assessing the health care needs of individuals and for the formulation of a nursing plan of care within the virtual, multimodal PC-2 (pre)habilitation hub, its implementation, evaluation of care and analysing variances/ responses to care delivery throughout each care episode.
•Provides clear instructions and guidance on interventions and use of the virtual hub to patients, carers and multidisciplinary team at Virtual Multimodal Hubs and participating sites.
•Appropriately coordinates with other Virtual Multimodal Hub or local sites as necessary for patient cover during absences.
•Assesses patients in a systematic way, identifies the patient's health problems and validates nursing plans of care by involving the patient and/or family in the plan of care. Develops appropriate teaching plans for the patient/family.
•Integrates information from the nursing assessment and from other health care team members.
•Revise the nursing plan of care appropriately to reflect current or changing status of the patient and evaluates as appropriate.
•Initiates referrals and utilises available resources within and outside the Institute for the purpose of comprehensive care to patients and families.
•Adheres to the principles of defensible documentation and ensures that nursing documentation is complete, up to date, signed and dated.
•Throughout the patient's hospitalisation promotes continuity of care, incorporates discharge planning in the nursing plan of care and records a discharge summary in the patient's nursing history at the time of discharge.
•Assists in maintaining a safe environment for patients, staff and visitors.
•Acts as a patient/carer advocate in relation to the patient/carer rights to all aspects of care, knowledge and privacy.
