About the Role
The Patient Navigator is a vital member of the Medical Oncology team and serves as the first point of contact for patients, carers, and community referrers.
This role is central to ensuring a smooth and supportive patient journey by:
• Responding to incoming calls and enquiries
• Assisting with appointment bookings, changes, and clarifications
• Supporting patient flow through the cancer care system
• Facilitating timely access to care
Key Responsibilities
As the Medical Oncology Precinct Patient Navigator (Breast Tumour Stream), you will:
• Coordinate referrals for breast medical oncology patients across both hospital sites
• Arrange internal cross-precinct referrals as required
• Coordinate treatment review bookings for Royal Melbourne Hospital patients attending day therapy
• Act as a key contact for breast medical oncology patients across the precinct
• Rebook patients when clinics are cancelled due to clinician leave
This is an important coordination role that directly supports high-quality, patient-centred cancer care across the precinct.
To be successful in this position you will require the following key attributes:
• Excellent communication and customer service skills
• Competent IT skills, experience with Epic is highly desirable
• Capable of working independently and in a team
• Able to prioritise work and to work with minimum supervision and guidance.
• Responsible and adaptable
• Demonstrated attention to detail with well-developed verbal and written communication skills.

Position Summary
At the Peter MacCallum Cancer Centre our patient care is delivered via a tumour stream model. This means that a patient’s care is coordinated and provided by a multi-disciplinary team of people who have specialist knowledge of a specific type of cancer. Medical Oncology is a precinct service with specialist clinics at both Peter Mac and Royal Melbourne Hospital.
The Patient Navigator is an integral member of this team, playing an important role in providing the first line response to incoming calls from patients, carers and referrers in the community needing assistance. The Patient Navigator resolves questions and concerns about how to organise appointments, change appointments or clarify appointment instructions
They are an expert in supporting patient flow through the cancer care system and play an important role in facilitating timely patient access.
Medical Oncology Precinct Patient Navigators are responsible for coordinating patient care in Breast and Neuro-Oncology for medical oncology patients. The role will co-ordinate referrals for breast and neuro medical oncology patients at RMH and PMC, organise internal referrals for cross precinct appointments as required, co-ordinate treatment review bookings for RMH
based patients with day therapy, be a point of contact for medical oncology patients across the precinct, rebook patients when clinics are cancelled for clinician leave.
The Patient Navigator works closely with the breast and neuro-oncology patient navigators, specialist nurses across the precinct.

Key Accountabilities
Provide accurate and efficient administrative support and coordination for precinct medical oncology patients.
- Coordinate referrals for medical oncology breast and neuro-oncology patients across the precinct.
- Coordinate treatment review bookings for RMH based patients
- Manage rebooking of patients for clinician leave for medical oncology breast and neuro-oncology patients for the precinct.
- Assist with rebooking requests, script requests and other administrative functions for breast and neuro oncology precinct patient
2. Provide exceptional customer service
- Provide a high level of customer service to patients, carers and staff within a designated team or portfolio
- Obtain the required information from patients, carers and staff in an efficient and courteous manner
- Respond to patient and/or staff queries in a timely and respectful manner
- Ensure phone and email enquiries are triaged to the correct destination.

3. Monitor and improve team performance
- Liaise with direct line manager to trouble-shoot any technical issues experienced by the work group in a timely manner to ensure quality is maintained
- Manage patient records in accordance with hospital policy and Data Accountability Framework to maintain data integrity at all times
- Ensure that the performance of the team meets the standards required by the organisation through a regular system of auditing performance
- Asses situations or information, clearly and accurately communicate information and make recommendations to line management
- Identify opportunities for process improvement and lead the team to implement appropriate solutions with support from the line manager
4. Support the work of the team and department

Work cooperatively as part of the team and assist other areas and staff as necessary

Ensure senior departmental staff receive adequate administrative support with rostering, meeting agendas and minutes, room bookings and database management.

5. Provide reports to line manager as requested and maintain key relationships

Provide routine and/or ad-hoc reports as requested by the line manager

Maintain relationships with external providers contracted to support Peter Mac patients
