YOUR ROLE IN OUR FUTURE 
Clinical Nurse Consultant - Allogenic Bone Marrow Transplant
Fixed Term, Part - Time Role.
 
The CNC AlloBMT is a part -time, fixed term position at 32 hours per fortnight. Reporting to the Specialist Nursing Team - Nurse Manager, The CNC AlloBMT the multidisciplinary team across Peter MacCallum Cancer Centre and Royal Melbourne Hospital to ensure optimal patient outcomes for patients across Peter MacCallum Cancer Centre and Royal Melbourne Hospital to ensure optimal patient outcome for patients accordingly.
 
This position will hold an honorary appointment at RMH.
 
•    Reviews AlloBMT patients newly discharged from 7B RMH at Peter Mac in collaboration with nursing and medical team.
•    Reviews AlloBMT patients preparing for discharge from 7B RMH as required.
•    Undertakes advanced clinical assessment of AlloBMT patients early post allograft and development of management plan in collaboration with medical team, patient & family.
•    Provides telehealth follow up for patients as required
•    Provides ongoing education and support for patients and family post allograft
•    Coordinates specialist and allied health referral as required
•    Participates in post allograft exercise programme in conjunction with physiotherapist and other AlloBMT specialist nurses
•    Provides educational and clinical support to staff and patients in wards
 
Remuneration as per Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020-2024. ZJ4: $122,907.20 (full-time equivalent - to be prorated for part time).

Position Summary

The CNC AlloBMT will work with the AlloBMT team to provide high quality expert assessment and management of patients post AlloBMT – and work collaboratively with the multidisciplinary team across Peter MacCallum Cancer Centre and Royal Melbourne Hospital to ensure optimal patient outcomes for patients across the Parkville precinct.

The CNC AlloBMT:

  • is an expert practitioner who uses and facilitates evidence based practice that is visionary, creative and appropriate to scope of practice and patient needs.
  • identifies, facilitates, provides, and evaluates educational processes to enhance the knowledge and competencies of health professionals.
  • Reviews AlloBMT patients newly discharged from 7B RMH at Peter Mac in collaboration with nursing and medical team.
  • Reviews AlloBMT patients preparing for discharge from 7B RMH as required.
  • Undertakes advanced clinical assessment of AlloBMT patients early post allograft and development of management plan in collaboration with medical team, patient & family.

  • May be required to attend some allograft long-term follow-up (LTFU) clinics.
  • Attends, participates, and presents at relevant Multidisciplinary Meetings and Journal clubs
  • Participates in leadership, research, and educational opportunities relevant to clinical haematology, AlloBMT and professional nursing.
  • It is expected that the AlloBMT CNC is active in professional organisations, leads and participates in projects and is active in conference presentation.

Essential Requirements
  • Registered Nurse (AHPRA).
  • Post graduate qualifications in cancer nursing.
  • Minimum 5 years’ experience in Haematology nursing, preferably Allogeneic BMT.
  • Demonstrated understanding of and experience with developing evidence-based practice and participation in projects, research.
  • Demonstrated participation in health professional and patient/carer education
Desirable Requirements
  • Completion / or working towards completion of Masters degree in nursing or related field.
  • Demonstrated involvement in professional activities and organisations.

  • Facilitation of coordinated and timely readmission for unwell patients through Same Day Medical Unit or RMH Emergency Department

  • Achievement of timely nurse lead review of Allograft patients in SDMU
  • Active reduction in unnecessary face to face visits through care coordination e.g. telephone reviews

3. Screening and Assessment

 

  • Referrals are triaged within 72 hours of receipt
  • Evidence based assessment tools are completed for new patients and referrals made as required
  • Demonstrates expertise in clinical assessment skills and care planning for patients with complex physical and psychosocial needs
  • Supports other nurses to develop expert clinical assessment skills and develop individualised care plans

4. Improving Quality and Safety of our services

 

  • Evidence of involvement in the development and/or recognition and/or evaluation of evidence based patient education resources
  • Evidence of delivery of high quality education
  • Evidence of participation in committees and relevant professional groups.
  • Collection of nurse sensitive KPI data that can be used to inform practice change. Examples may include the following:
    • Percentage of new patients screened for supportive care needs at initial consult
    • Number of phone calls received regarding symptom management
    • Number of patients who receive a survivorship consult at the end of treatment
    • Appropriate triage into PAC
    • Number of cancellations on day of treatment as patient is not ready for care
  • Evidence of contributing to/being involved in at least one research/QA project per year.
  • Evidence of submitting an abstract for conference presentation or manuscript for publication, minimum 1 submission every 2 years.
