About the Opportunity
Peter MacCallum Cancer Centre is seeking a passionate and dedicated Diabetes Nurse Educator to join our growing team. This is an exciting opportunity to contribute your expertise within a world-leading cancer centre known for excellence in patient care, research, and innovation.

In this specialised role, you will support patients living with or newly diagnosed with diabetes, ensuring they receive the knowledge, skills, and confidence needed to manage their condition effectively. You will work across both inpatient and outpatient settings, making a meaningful difference to patient outcomes during some of the most important moments of their lives.

 

What You’ll Be Doing
As a key member of our multidisciplinary team, you will play a vital part in ensuring the delivery of high-quality, safe, and responsive diabetes nurse education. Your responsibilities will include:

•    Providing individualised diabetes self-management education to patients and carers across inpatient wards and outpatient programs.
•    Acting as an expert clinical resource—offering guidance, support, and best-practice knowledge to patients, nurses, and broader clinical teams across Peter Mac.
•    Collaborating closely with clinicians and allied health professionals to deliver flexible, patient-centred care that responds to diverse needs in a dynamic clinical environment.

Position Summary

The Diabetes Clinical Nurse Consultant (CNC) will co-ordinate, negotiate, procure and manage the transition of care of the selected patient population across the health care continuum. The Diabetes CNC will act as a resource person, for the whole hospital, promoting best practice in the provision of patient care through clinical initiatives and service innovation. The Diabetes CNC will demonstrate superior clinical expertise together with outstanding interpersonal negotiation and communication skills to assist with the achievement of patient centred goals and the goals of the designated department and organisation.

Skills

Essential Requirements
  • Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
  • Post graduate qualifications in Diabetes Education and Management
  • Credentialed as a Diabetes Educator with the Australian Diabetes Educators Association (or working towards)
  • Demonstrated ability to work independently and practice collaboratively as part of the multidisciplinary health care team.
  • Advanced assessment, problem solving, critical thinking and organisation skills
  • Excellent communication and interpersonal skills
  • High level of reliability and professional conduct
Desirable Requirements
  • Experience in Insulin dose titration in the Inpatient and Ambulatory Care setting in patients undergoing high dose glucocorticoid therapy, TPN and PEG feeds.
  • Experience in Continuous Subcutaneous Insulin Infusions (Insulin Pump Therapy) initiation and management
  • Experience in Continuous Glucose Monitoring Systems, initiation, data upload and management.
  • Experience with Bolus Advisor Meters
  • Experience in using diabetes management technology and associated computer programs.
  • Experience with working in the inpatient and outpatient clinic settings.
  • Experience with the area of research and quality improvement
  • Experience in review and development of policies and procedures.

    1.1. Ensure the delivery of high standard of evidence-based nursing care to patients. 
    1.2. In the clinical setting, conduct comprehensive screening, assessment planning implementation and evaluation. 
    1.3. Establish and maintain effective communication with other health professional and support staff, patients’ families, the community and external care providers.
    1.4. Collaborate with other health professionals to ensure the patient’s safe and effective transition across the care continuum and facilitate access and referrals to appropriate health resources and services. 
    1.5. Lead and facilitate the development of systems and processes to support the planning and coordination of treatment, education and support needs for patients with cancer, and their families across the disease trajectory 
    1.6. Support the ongoing development of the clinical services model and provide support and information to nurses in the preceptorship program allocated to the clinical service 
    1.7. Actively engages in the preceptorship program, ensuring succession planning for the role 
    1.8. Contribute to the development of local, national and international evidence based care guidelines, policies and procedures and patient resources. 
    1.9. Support and promote excellence and autonomy in clinical practice and patient care

  • Coordinate the assessment, development, implementation and monitoring of a comprehensive plan of care from admission to discharge and in the outpatient setting in conjunction with the multi-disciplinary team.

    3.8. Demonstrates flexibility and creativity in identifying resources to meet patient care needs
  • Evidence of involvement in the development and/or recognition and/or evaluation of evidence based patient education resources
