About the Team
Peter Mac @ Home (PM@H) is the acute home-based oncology service for Peter Mac patients/clients and is available 7 days per week, 365 days a year. The service incorporates Hospital in the Home (HITH) and Home-based cancer care as funded through the DHHS.
 
About the Role
The PM@H unit is currently seeking an experienced ANUM to join our team in a part time position. This role requires a strong commitment to delivering exceptional community-based care, supporting the professional development of self and others, and providing evidence-based nursing practice. We are looking for a clinician who is confident working both autonomously and collaboratively within a multidisciplinary environment, while supporting the Nurse Unit Manager (NUM), fellow ANUMs, and broader team.
 
Key Responsibilities
Responsibilities include, but are not limited to:
•    Day-to-day staff coordination and management 
•    Liaising with key stakeholders, clinicians and multidisciplinary teams across Peter Mac 
•    Systemic Anti-Cancer Therapy (SACT) preparation coordination 
•    Referral management and processing 
•    Communication and coordination with patients, families and carers 
•    Portfolio and quality improvement work 
•    Supporting staff wellbeing and team culture 
•    Acting in the NUM role as required
 
About You
To be successful in this role, you will have:
•    Current AHPRA registration as a Registered Nurse 
•    A current and valid driver’s licence 
•    Minimum five years’ post-registration nursing experience 
•    Experience working within acute Hospital in the Home (HITH) settings 
•    Acute medical oncology and/or haematology nursing experience 
•    Strong leadership, communication and organisational skills 
Experience in surgical nursing and/or previous ANUM experience is highly desirable.

1. Management of the Patient Care Continuum

-Attends unit/department/divisional monthly meetings as required for the NM

-Attends monthly ward/department meetings

- Patient documentation is correctly completed 100% of the time 

  • Facilitates and organises care which maximises nursing expertise and meets all patients’ needs.
  • Promotes a multi-disciplinary approach to care.
  • Interacts with clinical nurses to assess the care provided and to develop clinical nurses’ abilities in providing care.
  • Ensures that documentation of nursing plans of care/care maps, variances and outcomes of care conform to legal requirements.
  • Supports and participates in ongoing education and research programmes of Nursing Services and the Organisation.
  • Supports systems that allow for consistent evaluation of nursing care provided e.g. Quality Assurance activities, peer review and case consultation.
  • Responsible for unit management in conjunction with other members of the management team.
  • Ensures that the daily operational activities of the ward related to supplies, equipment, and physical environment are attended to. 
  • Ensures that all staff are aware of and adhere to hospital policies. 
  • Ensures that staff maintain working knowledge of emergency procedures and location of emergency equipment.
  • Liaises with other members of the management team with respect to staff recruitment, budget considerations, and critical issues concerning patients, staff or the environment of care. 
  • Assesses all patients and staff related incidents and takes actions where appropriate; reports incidents to the NM and makes recommendations for change where warranted. 

2. Resource Management 


- Ensures 100% of staff perform at a level suitable to patient care requirements 
- Patient dependency completed on night shift 
- Evidence that 100% staff complete ward/dept based orientation 
- Evidence that 100% staff attend compulsory in-services including emergency procedures, BLS/ALS training 
- Evidence that 100% staff receive allocated preceptor 
- Follows up on reported incidents Financial and material 
 

  • Responsible for personnel management in conjunction with other members of the management team.
  • Coordinates the work of Registered Nurses Division One (1), Patients Services Assistants, and Ward Clerks/Administrative Support as necessary.
  • Manages available resources efficiently.
  • Liaises with staff regarding rostering arrangements and coordinates the ward/dept roster as necessary. 
  • Acts as a resource and support person for staff and other professionals. 
  • Coordinates the orientation of new staff members by organizing suitable preceptor/preceptor combinations.
  • Participates in staff recruitment, which includes developing criteria for personnel selection and an involvement in personnel evaluation through interview and consideration of professional work histories.
  • Participates in regularly scheduled staff meetings and conferences.

3. Clinical Practice, Education and Research 
- Ensures ongoing in-service program 
- Demonstrated involvement in evidence based practice, research, education or training through at least one project 
- Ensures nursing practice reflects current policies and procedures
  • Participates in the development of formal and informal educational sessions to assist nursing practice.
  • Collaborates with the Nurse Manager, Clinical Nurse Specialists, Professional Development Nurse and clinical nurses to determine priorities for educational sessions to assist nursing practice.
  • Conducts in-service education sessions  Encourages staff attendance at hospital in-service programmes and where possible, at external conferences.
  • Supports nursing research to promote clinical knowledge development and evidence based clinical practice. 
  • Systematically evaluates own practice and identifies pertinent areas of clinical practice management and education for investigation.
  • Ensures clinical practice is based on best practice standards in oncology. 
4. Professional Leadership 
- Evidence of active maintenance of own Professional involvement.
  • Responsible for maintaining competencies in those activities that contribute to the ongoing development of self, profession and other health professions.
  • Adheres to the Nurses Code of Ethics and functions within legal requirements affecting nursing practice.
  • Works according to established Nursing Services/Unit philosophies and objectives. 
  • Appraises own performance against this job description at least annually.
  • Contributes and represents the Unit or Nursing Services through membership of Committees and portfolios. 
  • Ensures all nursing staff have access to relevant and current information pertaining to nursing practice and the nursing service.
